Office Hours

For information or to schedule an appointment, please call us at 310.997.2383. We are open Monday through Friday from 8:30am to 5:00pm at our Santa Monica location.


Appointment Policy

We have a $50 cancellation fee for not cancelling the appointment within 1 business day. We confirm your appointment 24 hours in advance and hope you call back to confirm so we can run as efficiently as possible.


Insurance

Dr. Miller is an in-network provider for Blue Shield, Blue Cross, Cigna, and United Healthcare PPO plans. She is also a Medicare provider accepting PPO plans only. She will gladly accept all other PPO plans as an out-of-network provider and submit billing to your insurance company on your behalf.

Please note that due to ACA (Affordable Care Act), there are exclusions for each PPO network. In order to verify that Dr. Miller is in your network, we recommend that you call your insurance company to verify that her name appears under your plan.


Billing Policy

It is customary to pay all deductibles, co-insurance, and/or co-payments at the time of service. It is illegal for us to waive these charges. If your insurance cannot be verified at the time of service, you may reschedule your appointment or make full payment and a refund will be issued after the claim is billed and paid. It is your responsibility to know your benefits and bring proper valid documentation. We are unable to regularly verify benefits due to time constraints. If you have any questions about your bill, please call Alexsys Medical Management at 818.708.6848.

The following forms can be downloaded to your computer and printed at home. By completing these forms ahead of time, you will save time during your visit. Please bring these forms with you on the day of your scheduled appointment.

What to Bring to your Appointment

  • New patient packet and associated documents
  • Driver’s License
  • Insurance Card
  • Any previous radiology reports and CDs/Films
  • Any previous operative reports, EMG reports or other physician consultation relevant to your primary medical issue.
